Our experienced nurses play a hands-on role in caring for patients of all ages and stages. They specialize in different areas of care to meet the needs of our patients. They focus on assessment, prioritization and referral to the appropriate level of care.
Our advanced care team utilizes goal setting, identification of obstacles, and use of personal support systems. Our registered nurses work with patients in conjunction with their personal physician, to help them understand their medications and conditions. They provide education on lifestyle changes that benefit patients and allow them to take charge of their health care.
Our early intensive diabetes education and management helps patients optimize glycemic control to reduce risks that could complicate pregnancy. Our team assist patients in obtaining diabetic supplies and can refer patients to community resources. They educate diabetic patients on blood glucose monitoring, pattern management of blood glucose, insulin administration and safety guidelines if needed. Insulin pump and continuous glucose monitoring system training can also be provided.
Our mental health counselor works with people from all walks of life to identify goals and potential solutions to problems which cause emotional difficulty. She works with patients to improve communication and coping skills, strengthen self-esteem and promote behavioral changes and optimal mental health.
Counseling is an opportunity to receive support and experience growth during challenging times in life. Counseling can help one deal with many personal topics in life such as anger, depression, anxiety, substance abuse, marriage and relationship challenges, parenting problems, school difficulties, and adjustment to life changes.
